Horizon House’s four Community Residential Rehabilitation programs (CRR) in Chester County provide a structured living environment with 24-hour staff supports for people with mental illness (and sometimes a co-occurring drug and alcohol addiction). In all of them, staff works in partnership with the residents to promote their acquisition of independent living skills. Horizon House group homes are located in Phoenixville and Chester Springs. All referrals must come through the Chester County Office of Mental Health/Mental Retardation, 610-344-6265.
Horizon House also has a Supported Living Arrangement program that provides a range of services and supports to individuals with psychiatric disabilities (i.e. managing their medications, budgeting, interacting with neighbors and roommates, using community resources) to enable them to stay in their own homes.
