Horizon House Delaware County has developed a Program of Assertive Community Treatment (PACT) team called HOPE (Healthy Opportunities for People Everyday) as part of a joint effort with the Delaware County Office of Behavioral Health and Magellan Behavioral Health. The HOPE program is a best practices recovery model of service delivery that supports people with severe and persistent mental illness in the community. The HOPE team promotes recovery through comprehensive treatment, rehabilitation and support services. Individuals served by HOPE work in partnership with team members and learn how to manage their own lives, reach their individual goals and increase their personal happiness, success, and satisfaction. The HOPE program uses a multidisciplinary team (psychiatrist, nurses, specialists in the areas of mental health, substance abuse and employment, and peer specialists) to provide all treatment and rehabilitation supports to consumers with either a major mental illness, or a major mental illness and a co-occurring substance abuse disability.
