Philadelphia County Mental Health Services
Housing

Horizon House offers several transitional and permanent housing options in Philadelphia for people with mental illness (and sometimes a co-occurring drug and alcohol addiction). In all of them, staff works in partnership with the residents to promote their acquisition of the independent living skills required to maintain their connections to treatment and other supports in the community:

  • Licensed congregate housing that provides a structured living environment with 24-hour staff supports;
  • Clustered apartments that provide semi-independent living environments with staff-supports; and
  • Supported independent living options for individuals able to live in their own apartments.
All referrals for specialized housing supports must come through the Philadelphia Office of Mental Health’s Access to Alternative Services (AAS) Unit, 215-546-0300.

For more information call 215-386-3838.
