Horizon House offers several residential service options in Delaware County for people with mental illness (and sometimes a co-occurring drug and alcohol addiction). In all of them, staff works in partnership with the residents to promote their acquisition of independent living skills. Transitional and permanent living arrangements range from a highly structured group home, to transitional housing, and staff-supported independent living options.

Horizon House’s full-care Community Residential Rehabilitation (CRR) program located in Prospect Park, provides a structured living environment with 24-hour staff supports. All referrals to this program must come through the Delaware County Office of Behavioral Health, 610-713-2365.

A Supported Living Service in Landsdowne consists of clustered-apartments with on-site staff. Staff provides a range of services and supports to the individuals (i.e. rental assistance, managing their medications, budgeting, interacting with neighbors and roommates, using community resources) to enable them to stay in their own homes. All referrals to this program must come base through the Delaware County Office of Behavioral Health, 610-713-2365.

In addition, Horizon House also provides a Supported Living Service consisting of clustered and scattered-site apartments throughout the county for individuals with mental health issues who are in need of non-time limited housing, and supports.
