Horizon House/Delaware is the Community Continuum of Care Program (CCCP) provider for New Castle County. A statewide initiative, CCCP is a comprehensive, integrated recovery approach to working with people diagnosed with psychiatric disabilities (and sometimes a co-occurring drug and alcohol addiction). By partnering with those using the service, CCCP service and resource coordinators (case managers) help to identify goals that are meaningful to the person’s recovery. They provide a comprehensive array of resource/service management supports that includes: clinical and rehabilitation services, employment and educational rehabilitation and supports, 24/7 crisis response, full-time psychiatrists, housing assistance, and supports for families. The CCCP approach allows individuals to remain within one service system in which all services are provided by the same staff.
