Service and Resource Coordination

Horizon House offers several different case management options, including:

Targeted Case Management (TCM)
The Targeted Case Management unit provides 24-hour, 7-day a week resource and service coordination to individuals with psychiatric disabilities and/or drug and alcohol addictions. Many of them also have physical problems (HIV/AIDS, diabetes, heart disease, etc.). Working in partnership with those using the service, TCM interdisciplinary team members identify treatment and rehabilitation goals that are meaningful to the person’s recovery. In addition to service managers, the TCM team includes a psychiatrist, nurse, drug and alcohol specialist, and benefits specialist. All referrals must come through the Office of Mental Health’s Targeted Case Management Unit,215-599-2150.

Community Support Services (CSS)

Individuals with mental illness who would like to live in a personal care boarding home may receive case management in the home through the Community Support Services (CSS) program. Boarding homes served by CSS team managers are not licensed, but the providers work closely with the staff to ensure that the residents receive coordinated services. All referrals must come through the Philadelphia Office of Mental Health’s Access to Alternative Services (AAS) Unit, 215-546-0300.
